Job Description
We are seeking a dedicated Staff Pharmacist to join our long-term care pharmacy team. The ideal candidate will provide personalized medication management services to residents in long-term care facilities, ensuring the safe and effective use of medications.
Key Responsibilities:
- Review and Interpret Orders: Evaluate physician orders for accuracy and appropriateness.
- Dispense Medications: Accurately dispense prescribed medications and maintain patient medication profiles.
- Monitor Therapeutic Responses: Assess and monitor patients' responses to medications.
- Identify and Resolve Issues: Address potential and actual drug-related problems.
- Provide Drug Information: Educate patients, caregivers, and healthcare professionals about medications.
- Collaborate with Healthcare Teams: Work with other healthcare professionals to develop and implement individualized care plans.
- Conduct Medication Reviews: Regularly review medication regimens for safety and efficacy.
- Ensure Compliance: Adhere to federal and state laws, regulations, and standards related to pharmaceutical care.
- Consult on Health Matters: Offer advice on health and wellness, including disease management.
Qualifications:
- Education: Doctor of Pharmacy (PharmD) or RPh
- Licensure: Valid and active pharmacist license in the state of Texas
- Experience: Previous experience as a pharmacist, preferably in a long-term care setting.
- Skills: Strong knowledge of drug administration, excellent organizational and communication skills, attention to detail, and the ability to work with an interdisciplinary team.
- IV Certified
Work Environment:
- Setting: Long-term care pharmacy (closed door)
- Schedule: May include shift work, evenings, and weekends.
- Reporting: Reports to the Pharmacist-in-Charge.

Pay: $55.00 - $58.00 per hour

Schedule:
- Rotating shifts - 9 to 6, 11 to 8 and 12 to 9
- 8-hour shift

- Every 5th Saturday
Experience:
- Clinical pharmacy: 1 year (Preferred)
License/Certification:
- Registered Pharmacist (Required)
- Licensed Pharmacist (Required)
